The Physical Well-Being Benefit 

Whether you use a participating facility or a non-participating facility, the Physical Well-Being Program is contingent upon:

​

  • The terms of your benefit plan.

  • Your eligibility for benefits.

  • Your continued employment for the duration of your membership or participation in the program.

 

Using the Physical Well-Being Benefit

Participating Facility

 

When you join a participating facility, simply notify the facility that you are a participant of the UFCW Local 1776 and Participating Employers Health and Welfare Fund. You will be given an eligibility form which both you and the center will complete. The participating facility will then verify your eligibility with the Fund Office. Once your eligibility is verified, you will then complete a quarterly utilization report to track your usage for each of the four calendar quarters.

At many participating facilities, the Fund’s allowance will be considered payment in full. If, however, the participating facility requires that you make additional payments, you should confirm the specific amount and conditions of your co-payment at the time you join the facility. Call the Fund Office to verify this amount.

How much is covered? 

Single Membership: $300 allowance per calendar year

Family Membership: $500 allowance per calendar year

Note: Part-time employees are covered for a single membership only (no dependent coverage)

Non-Participating Facility
 

When you join a non-participating facility, call the Fund Office to request reimbursement information. In order to receive reimbursement, you must submit the following items to the Fund Office for verification:

​

  • Completed reimbursement form indicating usage

  • Copy of itemized receipt and/or proof of payment

  • Copy of the membership agreement

 

If you sign up for an annual membership at a non-participating facility, you and the facility must complete the reimbursement form and can submit these items twice each calendar year (halfway through the program and again at the program's end).  If you join a program that is not utilized throughout the entire year (e.g., smoke cessation, weight loss, swim clubs, etc.), the reimbursement form and other information may be submitted when the program is completed, rather than twice each calendar year.

How much is covered? 

Single or Family Membership: $250 reimbursement per calendar year

​

 

Advantages of Using a Participating Facility 

​

You receive a higher benefit when you join a participating facility instead of a non-participating facility: You can receive a benefit of up to $500 at a participating facility, while the maximum benefit is $250 at a non-participating facility.

The Fund pays the participating facility directly; so the amount of your cost, if any, is reduced. With a non-participating facility, you must the facility and then submit a claim for reimbursement.

A participating facility has a contract with the Fund which guarantees its membership rates. A non-participating facility can charge whatever it wants and can change its rates at any time throughout the year.​​
